What varies is whether people believe in the process or just comply with it. Compliance means the form gets filled out. Belief means someone actually stops and asks what could go wrong today, with this crew, in these conditions? In a busy operational environment with schedule pressure from above, that takes real discipline.
Risk Assessment in oil and gas is not a desk exercise. It has to reflect what is actually happening on the ground. The moment it becomes a signature-gathering exercise, it stops being a control and starts being a liability document.
Here is something I have observed consistently: hazard identification almost always gets done too fast.
Not because people are lazy. Because it feels like the job has not started yet, so there’s pressure to move through pre-work steps and get on with it. That pressure usually unspoken, sometimes explicit is where a lot of incidents are born.
Good hazard identification is a conversation, not a checklist. It asks uncomfortable questions. What if the isolation fails? What does this job look like if the wind picks up? Done right, it breaks a task into steps, names the hazard at each one, and documents what control reduces it. That takes time and it consistently feels like a delay, right up until something goes wrong and everyone wishes the conversation had happened.
The version done for show looks identical on paper. Steps listed, hazards named, controls written in. What did not happen is the actual discussion where someone says, “the drain valve on that line has been leaking” or “the wind today puts us downwind of the flare.” Those moments only happen when a JSA is treated as a planning tool, not a form. That starts with whether supervision actually models the behavior.
This is one area where Critical Risk Management thinking has genuinely improved how the industry approaches risk controls the recognition that not all controls are equal.
Some controls, if they fail, create inconvenience. Others the Safety Critical Controls are the last barriers between a hazardous energy release and people getting killed. Gas detectors. Emergency shutdown systems. Physical isolation points. Pressure relief devices.
These do not get treated like general controls. They get verified physically, before work starts, and periodically throughout the job. An isolation that has not been tested is not a barrier. A gas detector that was not calibrated this week is not a safety critical control. It is a false sense of security with a display on it.
Organizations that explicitly identify and verify safety critical controls before sign-off have fewer major incidents. That is not theory it is observable across decades of industry incident data.
Process Safety is where all of this connects to something larger. It is about the major hazard risks pressures, temperatures, substances that if released, do not just injure one person. They destroy infrastructure and kill multiple people.

Not as the only cause, but as factors a more disciplined system might have caught.
